General Requirements:

1.  Passport: A valid passport with at least six months of remaining validity and at least two blank pages.
2. Photographs: Recent passport-sized photographs with a white background (as per the specifications mentioned on the visa application form).
3. Visa Application Form: Completed and signed visa application form, which can be filled out online or obtained from the Indian embassy or consulate.
4. Proof of Medical Appointment: A letter from the recognized medical institution in India where the treatment is to be undertaken, detailing the medical condition and the necessity of the treatment. Admission letter from the hospital specifying the details of the treatment, estimated duration, and costs involved.
5. Medical Records: Copies of the patient’s medical records and treatment history.
6. Proof of Residence: Proof of residence or stay in the applicant’s home country, such as a utility bill or lease agreement.
7. Financial Proof: Bank statements or financial documents proving sufficient funds to cover medical expenses and stay in India.
8. Insurance: Proof of travel and medical insurance covering the duration of stay in India.
9. Previous India Visa Copies: If applicable, copies of any previous India visas.

  • Separate visa application forms for the attendants who will accompany the patient.
